ACE Questionnaire
Adverse Childhood Experiences(ACE) Questionnaire
ACE is a 10 question self-report measure, developed by the CDC in cooperation with Kaiser-Permanente, that rates the extent of trauma a person experienced during childhood. It is used to predict the likelihood that patient will experience one or more forms of health, behavioral, and/or social problems. Ace domains include:
Abuse (Emotional, Physical and Sexual)
Neglect (Emotional and Physical)
Household Dysfunction (Mother treated violently, Household Substance Abuse, Household Mental Illness, Parental Separation and Divorce, Incarcerated Household Member)
